Notes for Prospective Students

A completed application to SMA for grades 6-12 includes the following items: 1) application for admission and report of medical history, completed and signed by the student and custodial parent 2) $150 non-refundable application fee 3) transcript of grades with achievement and/or IQ test results if available 3) two teacher evaluations from the school last attended; one from an English teacher and one from a math or science teacher 4) a personal interview and campus visit with the applicant and parent(s). This requirement may be waived or modified for out-of-state or international applicants 5) copy of birth certificate and passport if applicable
