Notes for Prospective Students

> Neuropsychological or psychoeducational testing less than three years old. Our required tests/assessments are: WISC (Wechsler Intelligence Scale for Children) or WAIS (Wechsler Adult Intelligence Scale, 17yo/older); WIAT (Wechsler Individual Achievement Test) OR WJ (Woodcock-Johnson Tests of Achievement). If possible, we would also like to see the following:BASC (Behavior Assessment System for Children); ABAS (Adaptive Behavior Assessment System); BRIEF (Behavior Rating Inventory of Executive Function). > Most recent SLP testing, if the student receives speech services. > Copy of current IEP, if applicable. If the student has an FBA and/or a BIP, this must be submitted as well. > Transcripts (grades 9-12) and/or report cards (grades 6-8) > Photo
